WIHA forms will be placed here in Adobe PDF format. You will need Adobe Acrobat Reader (which usually comes with your browser) to view, fill-out and print these forms. If you need the reader, you can get it here. WIHA Registration Form : You can fill out this form on line, and print it to your printer. Then, mail or deliver it to league officials at 258 Palm Ave., Woodland, CA 95695 with your payment. To fill out the form, simply click the "WIHA Registration Form" link above, select the "hand" tool in Adobe (it looks like a hand) and "tab" or "click" through the yellow form fields, supplying the necessary information, then select the print option. The printed version will have all of the fields filled out as you did on the screen. If you make a mistake, just go back and delete the information and try again. |
